About Semaglutide

Semaglutide is a synthetic analogue of the human incretin hormone GLP-1, structurally modified with a C18 fatty-diacid chain that extends its plasma half-life to around a week. It is one of the most-studied peptides in the incretin class and the reference point for single-receptor agonism in metabolic research.

How Semaglutide works

Semaglutide is a GLP-1 receptor agonist — an analogue of the human incretin hormone GLP-1, sharing most of its sequence. Two amino-acid substitutions make it resistant to enzymatic degradation, and a C18 fatty-diacid chain attached through a short linker promotes binding to serum albumin.

That albumin binding is what stretches the plasma half-life to roughly a week, which is why published work uses a once-weekly rather than a daily schedule. It engages a single incretin receptor — the reference point against which the dual- and triple-agonist molecules that followed are measured.

What the research shows

Semaglutide was evaluated in the STEP programme of randomised trials in overweight and obesity, and in the SUSTAIN programme in type 2 diabetes. A 2025 randomised trial compared it head-to-head with tirzepatide.
