NAD+ has two roles, and only the second one explains why a precursor is studied at all. The first is as a redox carrier, cycling between NAD+ and NADH to shuttle electrons through glycolysis, the citric acid cycle and oxidative phosphorylation. A carrier is regenerated, so it is not used up.
The second is as a consumed substrate. Sirtuins, PARPs and CD38 cleave NAD+ as part of their catalytic cycle. That consumption is what ties the cellular concentration of NAD+ — not merely the NAD+/NADH ratio — to deacetylation signalling, DNA repair and calcium signalling, and it is why the supply side of NAD+ is studied in its own right.
